1. Fill out the form
2. Obtain Rameen Talesh Ed.D., Associate Vice Chancellor SLL’s signature as the next level of authority.
3. Obtain the signature of the VC, Dean, or Chief, Dr. Willie Banks, Vice Chancellor, Student Affairs
4. Utilize DocuSign to collect all the required signatures.
5. Once the form is completed, inform Karina Cruz, Sr. Financial Analyst for review. She will check for sufficient funding in the account.
6. After she confirms the funding, send it to the HR Representative, Sunny, including the signed Nomination Form and STAR Award Letter.
